Sehat Sahulat Card Eligibility Criteria:
- To check your program eligibility, SMS the number on your National Identity Card to 8500.
- If you are eligible, you can collect your Sahat Insaf card from the card distribution center in your area.
- Being a citizen of the country.
- Coming under low income family or being a part of certain government schemes.
- Registration with relevant authorities and compliance with necessary documentation requirements.

- Bring the following documents with you when you go to a government/private hospital with empanelment.
